✔️ How does Nictoris 20 work?

Nictoris 20 works by increasing blood flow through the heart vessels, leading to improved blood and oxygen supply to the heart muscle thus reducing the workload of the heart which results in reducing the frequency and severity of angina attacks.

✔️ Quick Suggestions:

• Exercise regularly. It helps in maintaining proper weight and lowers cholesterol.
• Limit salt and sugar intake.
• Avoid smoking and alcohol consumption.
• Nictoris 20 may cause headaches. Drink plenty of water and take a suitable painkiller. Inform your doctor if it does not go away.
• Nictoris 20 may cause dizziness or sleepiness. Do not drive or do anything which requires concentration until you know how it affects you.
• Do not consume alcohol while taking Nictoris 20 as that may cause increased dizziness.
• Do not take medications like sildenafil or tadalafil (prescribed for erectile dysfunction) while on treatment with Nictoris 20 as it may cause low blood pressure.

✔️ Pharmacology

Nicorandil works by dilating arterioles and large coronary arteries through the opening of potassium channels. Additionally, it stimulates guanylate cyclase, leading to venous vasodilation. These actions collectively reduce preload and afterload on the heart and enhance coronary blood flow.

✔️ Dosage & Administration:

Adults:

• Usual Therapeutic Range: 10 to 20 mg twice daily.
• Starting Dose: 10 mg twice daily (morning and evening). This dose can be increased based on patient needs, response, and tolerance, up to a maximum of 40 mg twice daily.
• Lower Starting Dose: 5 mg twice daily may be considered for patients particularly prone to headaches.

Elderly:

• There are no special dosage requirements for elderly patients.
• The lowest effective dose should be used, and it should be administered with care, starting with lower dosages.

Children:

• Nicorandil is not recommended for use in children and adolescents under 18 years of age.

Patients with Hepatic Impairment:

• Nicorandil should be used with caution in patients with serious hepatic dysfunction.
• Dose reduction may be necessary depending on the severity of the hepatic condition.

✔️ Interaction

Interactions: While no significant pharmacological or pharmacokinetic interactions have been observed in animal and human studies with Nictoris (Nicorandil) when used with beta-blockers, calcium antagonists, digoxin, a combination of digoxin/furosemide, rifampicin, and cimetidine, the following should be noted:

• Potential Potentiation: Nicorandil may enhance the effects of other vasodilators, tricyclic antidepressants, and antihypertensive drugs when used concurrently, especially with alcohol.

✔️ Nicorandil is contraindicated in the following conditions:

• Hypersensitivity: Patients who have shown hypersensitivity to Nicorandil.
• Cardiogenic Shock and Acute Myocardial Infarction: Patients with cardiogenic shock or acute myocardial infarction with acute left ventricular failure and low filling pressures.
• Hypotension: Patients with hypotension.
• Phosphodiesterase-5 Inhibitors: Concurrent use with phosphodiesterase-5 inhibitors can lead to a serious drop in blood pressure.

✔️ Warnings and Precautions:

• Blood Volume Depletion: Use Nicorandil with caution in patients who may have blood volume depletion.
• Low Systolic Blood Pressure: Use with caution in patients with low systolic blood pressure (below 100 mm Hg).
• Mouth Ulceration and Stomatitis: Discontinue Nicorandil if mouth ulceration, stomatitis, or persistent/severe buccal ulcerations occur, and take appropriate measures.
• Glaucoma: Caution is advised for patients with glaucoma.
• Hypotensive Effect: The hypotensive effect of Nicorandil may be increased when taken with other vasodilators, tricyclic antidepressants, or alcohol.
• Combination with Antihypertensives: Nicorandil should be used with care when prescribed antihypertensive drugs as it may further lower blood pressure in hypertensive patients.
• Mutagenicity and Carcinogenicity: Animal studies have not revealed any adverse mutagenic or carcinogenic effects when Nicorandil is used under experimental conditions.
